Health and income-protection benefits
All teammates who are scheduled to work 30 or more hours per week are eligible to participate in Myriad Genetics’ health and income-protection benefits. You may also cover your eligible dependents under Myriad Genetics’ medical, prescription, dental, vision, and life benefits.
Your eligible dependents include:
- Spouse/domestic partner (same or opposite gender)
- Your child(ren) and the child(ren) of your covered spouse/domestic partner (up to age 26)
- Children with disabilities who became disabled on or before age 26
401(k)
You are immediately eligible to enroll in the 401(k) on your date of hire if you are over age 21. You may change your contribution rate and investment elections at any time.

How to Enroll
Health and income-protection benefits
You must enroll within 30 days of your hire date to have coverage this year. You have two ways to enroll:
- Online – Enroll on the Oracle Self Service website any time of day or night.
- By Phone – Call 1-888-224-0678 if you have questions or need assistance. Representatives are available to help you from 9 a.m. – 5 p.m. MST, Monday through Friday.
Do I need to enroll?
Yes, you MUST take action within 30 days of your date of hire to have the benefits you want in 2025. If you don’t enroll, you will automatically be enrolled in one of the following medical plans according to your location:
- If you live within a Regence BCBS HPN service area, you will be enrolled in the Regence BCBS HPN CDHP at Employee Only coverage.
- If you DO NOT live within a Regence BCBS HPN service area, you will be enrolled in the Regence BCBS Traditional CDHP at Employee Only coverage.

The Oracle Self Service website will guide you through the benefits enrollment process every step of the way.
Remember!
- Be sure to provide the necessary dependent information to enroll eligible family members.
- Make sure you’ve provided beneficiary information for your life insurance.
Don't miss out
If you don’t enroll within 30 days of your date of hire, you cannot make benefit elections until our Open Enrollment period, unless you experience a qualifying life event.
401(k)
You have two ways to enroll in the 401(k) to invest for retirement through easy payroll deductions:
- Online – Enroll on the Fidelity Investments website any time of day or night.
- By Phone – Call 1-800-835-5095 if you have questions or need assistance. Representatives are available to help you Monday through Friday.
You may change your contribution rate and investment elections at any time.

Make sure to provide beneficiary information for your 401(k) account.
